'Child custody' deals with the care, control, and protection of a minor child'. The 'temporary custody forms' are the agreement by two parties involved in the giving and accepting of the custody respectively of a minor child. It allows a person to be the guardian of the child for a fixed period of time, as mentioned and agreed to in the form.

A temporary custody form has the following main contents

• If the minor child gets some illness or injury, while the parents are away, the temporary guardian has a right to approve or decline any medical facilities to the child. This is called the 'healthcare power of attorney'. All the required insurance claims and other important documents should be processed while accepting the custody.

• The temporary custody holder has a right to make any decisions regarding the education, schooling, or any other activities.

• Under the law, the agreed temporary custody form will act as a 'caregiver authorization affidavit'

• The travel clause is for the acceptance to let the child travel anywhere with the temporary guardians. The parents can very well restrict to any kind of travel.

• Another important provision inclusive in the forms are the 'health insurance portability act' and 'accountability act'.

• The period of temporary guardianship should be fixed in the beginning itself.

• A separate form for ending the temporary custody of a child is provided along with the custody forms. This gives the parents of the child in custody, to end the custody whenever they wish to.

This form comes with a medical information form, according to which, a doctor needs to clarify and give all the medical details of the child whose custody is being taken. This includes details of any disease the child is suffering from, medications if any that the child is taking along with the insurance information and the contact information of the parents.
